You must complete this form and submit it with the appropriate licensure and initial registration fee ($371) directly to the Office of the Professions to apply for licensure in NYS. $371 is the total of the application fee ($175) plus the fee for your first registration period ($196). The application portion if the fee is not refundable. View the Profession Fee Chart at http://www.op.nysed.gov/documents/opfeechart.pdf. You must answer all questions and provide all information requested unless otherwise indicated. Failure to complete all required parts of the application will delay its review.

This is the application and fee for licensure as a LMHC. You must also meet requirements for mental health counseling education, supervised experience and NCMHE. You are responsible for having all other documentation submitted for review and approval by the Department.
You may not practice mental health counseling in New York except with a limited permit issued by the Department. Neither licensure in another jurisdiction nor national certification substitute for authorization by the Department.
If you have been licensed as a LMHC in another U.S. jurisdiction and practiced mental health counseling for at least 5 of the 10 years prior to application in New York, you may qualify for endorsement. You must meet all requirements listed on our website, including verification of licensed practice in the other jurisdiction and pass the NCMHCE.
