Applicants for Podiatry licensure must document successful completion of at least 60 US semester hours of preprofessional education (including courses in general chemistry, organic chemistry, biology or zoology, and physics) following an acceptable Doctor of Podiatric Medicine program. They must also pass an examination acceptable to the Department and at least one year of supervised postgraduate hospital training. You must complete this form and submit it with the appropriate licensure and initial registration fee ($377) directly to the Office of the Professions to apply for licensure in NYS. $377 is the total of the application fee ($135) plus the fee for your first registration period ($242). The application portion of the fee is not refundable. You must answer all questions and provide all information requested unless otherwise indicated. Failure to complete all required parts of the application will delay its review.
