An applicant for licensure in Clinical Laboratory Technologist must have either:
- a bachelor's or higher degree in clinical laboratory technology from a program registered by the Department or determined by the Department to be the substantial equivalent.
- a bachelor's or higher degree in biology, chemistry, or the physical sciences or the substantial equivalent AND an advanced certificate from a registered clinical laboratory technology program or a program determined by the Department to be the substantial equivalent.
To be determined the substantial equivalent, the clinical laboratory technology program must include ALL of the NYS specific required content, as further outlined at http://www.op.nysed.gov/prof/clt/clp-cltlic.htm#equiv1. Applicants must also pass the American Society for Clinical Pathology (ASCP) Board of Certification's examination for Medical Laboratory Scientist (MLS) for licensure as a Clinical Laboratory Technologist. In addition, applicants must have good moral character, as determined by the Department. By submitting an application for licensure, you acknowledge that you have read and understand the licensure requirements.
You must complete this form and submit it with the appropriate licensure and initial registration fee ($371) directly to the Office of the Professions to apply for licensure in NYS. $371 is the total of the application fee ($175) plus the fee for your first registration period ($196). The application portion of the fee is not refundable. You must answer all questions and provide all information requested unless otherwise indicated. Failure to complete all required parts of the application will delay its review.
